“We are pleased to welcome Dr. Kozak to our Village of Oak Creek primary care location,” said Northern Arizona Healthcare Medical Group President Pasquale Bernardi, MD. “Dr. Kozak has an impressive background in health care and we know that she shares NAH’s commitment to delivering high-quality care to every patient. We are excited to grow our group of primary care providers in order to deliver excellent health care close to home for Verde Valley-area residents,” Dr. Bernardi added.

Dr. Kozak, who has over 25 years of experience in health care, specializes in family medicine. She enjoys treating patients within the same family which helps her to understand if any trends or reoccurring health concerns exist among family members. Dr. Kozak believes in giving her patients the guidance, advice and education to help them make the best decisions about their personalized treatment plan.

Dr. Kozak earned her medical degree from the University of New England College of Osteopathic Medicine in Biddeford, Maine and her master’s degree in psychology from Rutgers University in New Brunswick, New Jersey. She completed her internship and residency in family practice at St. Luke’s Warren Hospital in Phillipsburg, New Jersey. Dr. Kozak is board certified in family medicine and is a member of the American College of Osteopathic Family Physicians (ACOFP) and the American Academy of Family Physicians (AAFP).
