    Northern Arizona Healthcare to Offer $10 Sport Physicals for Student-Athletes in Verde Valley

    VERDE VALLEY, Ariz. – All Verde Valley area high school and grade school student-athletes are invited to attend an extensive one-day sports physical clinic, brought to the community by Northern Arizona Healthcare’s (NAH) Orthopedic & Spine Institute, NAH Primary Care and EntireCare Rehab & Sports Medicine. This is the fifth year NAH has provided the low-cost screenings to the community. All student-athletes must complete a pre-season physical to participate in all sports, so the June event covers fall, winter and spring sports eligibility

    The clinic is from 3 p.m. to 5:30 p.m., Thursday, June 26, at the NAH Camp Verde campus, located at 1298 West Finnie Flat Rd. The screenings are $10, and no appointment is necessary. Cash payments or checks made out to Northern Arizona Healthcare will be accepted. All proceeds from the physicals are donated back to the student’s school to benefit the athletic program.

    The screenings require a signed release from a parent or guardian if the athlete is under the age of 18. If the athlete is participating in any contact sports, they must download the Sway Medical mobile app prior to their physical. Sway is an FDA-cleared app that reinvents the way athletes and patients are monitored for signs of imbalance or delayed reaction. It is available for download on Apple or Android devices.     Release forms are available at the NAH Camp Verde campus, or at this link: www.aiaonline.org/about/forms. Forms 15.7A and 15.7B should both be printed and brought to the clinic. Form 15.7A needs to be completed in full prior to attending the clinic. Form 15.7B needs to have the name, date of birth, age and sex sections filled out prior to the appointment

    The sports physicals will include internal medicine, baseline sway concussion testing and orthopedic clearance. The tests will be performed by internal medicine physicians, orthopedic physicians and surgeons, athletic trainers and physical therapists.
